custom pics?

I have seen some kool pics that have been customised i just wondered how you did it? e.g mrs lam and pooh bear and rob you gave nobby on of pudsey in a stains shirt would someone please tell me how you do it ? Thanks !! :D

I found that pic on another website, can't recall which site it was though, sorry!

I don't know how to expurrlain how I've done that pic! I just get strange ideas in my head and then I have to design them.
