I think they actually ended up making a bigger mistake. The TJ was speaking to Ganson before the knock-on took place, I think he was calling the forward pass. Then there was some confusion because Ganson hadn't seen it. He can't take advice on forward passes from the video referee though. They got that call wrong.

I think it is a fitness factor Rob for sure. It would be interesting to marry that to internal organs too such as heart rate / blood pressure and so forth. Some NRl teams and AFL in Aus now have equipment fitted to them that monitors their position on the field throughout the game, how far/fast they run and factors such as heart rate. I believe that certain individuals are more predisposed to make errors when the adrenalin etc is flowing and that not only should referees be fitter they should learn about stress management etc. Calm people seem to make better referees.
As you say players losing game handle pressure badly and referees are human too. The greater stress they are under the more likely they are to make errors.
I do think referees are very badly served by their touch judges. TJs are often in a far better position to see things such as forward passes.
Totally agree about fitness and concentration going together. also the referees ability to communicate reduce when they tire. I don't think that is a major issue at SL level though. Towards the end of a game everyone tires a little but referees, especially at SL standard, train to perform for 90-100 minutes so that they are not caught out late on.
Referees are getting more and more help from their touch judges in the last season or two. James Child and Clint Sharrad both give information to the referees. In the Warrington -v- Hull KR Cup match, James Child was the main reason so many forward passes were given. They both have fitness in common too, they are always up with play to spot things.
To suggest that TJs are told not to say anything is rubbish (I know you didn't suggest that CP).
